245D Waiver Programs
Individualized Home Supports (IHS) with Training
IHS with Training offers skill-building and instructional services designed to help adults gain, maintain, or improve the abilities needed to live more independently and meaningfully in their communities. These services include assistance with locating and utilizing community resources.
Each individual receives personalized training and support from a qualified Accra staff member who is matched to their specific needs. Support is provided in at least one of the following community living service categories:
- Community Participation
- Health, Safety, and Wellness
- Household Management
- Adaptive Skills
IHS with Training services are available to individuals enrolled in the following waiver programs:
- Brain Injury (BI) Waiver
- Community Access for Disability Inclusion (CADI) Waiver
- Community Alternative Care (CAC) Waiver
- Developmental Disabilities (DD) Waiver
Individualized Home Supports (IHS) without Training
IHS without Training offers support, supervision, and assistance to adults and children living in their homes. A trained staff member provides direct help with daily living activities, including supervision, cueing, guidance, instruction, and assistance with attending or coordinating community activities. These services can be delivered either at home or in the community.
This service was developed as part of Minnesota’s Waiver Reimagine project and combines elements of the former Personal Support and Adult Companion services.
IHS without Training is available to individuals enrolled in the following waiver programs:
- Brain Injury (BI) Waiver
- Community Access for Disability Inclusion (CADI) Waiver
- Community Alternative Care (CAC) Waiver
- Developmental Disabilities (DD) Waiver
Respite Services
Respite is a short-term care service designed to give a temporary break to an individual’s primary caregiver. Care can be provided in the individual’s or respite provider’s home. Respite services are available through multiple funding options, including medical assistance, home and community-based services (waiver programs), and private pay arrangements.
Homemaker Services
Homemaker services offer support with daily household tasks, ranging from light cleaning and laundry to help with managing the home and routine daily activities. Home management tasks may include:
- Preparing Meals
- Shopping for Groceries, Clothing, and Other Essentials
- Performing Simple Household Repairs
- Coordinating Transportation
These services are available through various funding options, including medical assistance, home and community-based services (waiver programs), and private pay.
Night Supervision Services
Night Supervision provides overnight assistance and monitoring by a staff member in the client’s home. This service is designed to support clients who have an assessed need in any of the following areas:
- Assistance with Activities of Daily Living (ADLs)
- Support with Instrumental Activities of Daily Living (IADLs)
- Implementation of Positive Support Programs and Transition Plans
- Reinforcement of Skill Development Strategies
Individual Community Living Supports (ICLS)
ICLS aids older adults who need reminders, cues, occasional or moderate supervision, or physical help to continue living independently in their homes. ICLS services are available to eligible individuals 65 and older and cover support in the following areas:
- Active Cognitive Support
- Adaptive Support Services
- Assistance with Activities of Daily Living (ADLs)
- Household Management
- Health, Safety, and Wellness
- Community Engagement
ICLS services are available to individuals enrolled in the following waivers:
- Elderly Waiver
- Alternative Care Waiver
Assisted Living
